Treatment of Refractory Adult Immune Thrombocytopenic Purpura with rotein A-immunoadsorption.



Abstract
Extracorporeal immunoadsorption of plasma to remove IgG and circulating immune complexes (CIC) employing staphylococcal protein A column (ProsorbaTM) was evaluated as a therapy for five female patients with refractory immune thrombocytopenic
purpura
(ITP). One patient showed allergic vasculitis after the first trial and further immunoadsorption was not performed. The other patient had a difficulty in vascular access and only three immunoadsorption were performed. Among the three patients who
received more than 7 immunoadsorptions, one patient showed dramatic increase of platelet count 10 days after the last procedure which was maintained more than 5 months. A relapse followed but was controlled by the administration of prednisone and
danazol. A decrease in the platelet antibody titer and the CIC was also observed during the immunoadsorption in this patient. The other two patients didn't show any improvement after the immunoadsorption procedures.
Authors concluded that in some selected cases with refractory ITP, the use of immunoadsorption could produce a lifesaving dramatic improvement.
